I wear the icecap smooth side out - I think this makes it a little easier to have some water circulate enough to pressurize the ears - and helps the mask seal - it also matches my Cressi Matrix perfectly. spearfishingear.com is where I got it - took them a couple days to get it but no big wait. Wonderful piece of kit - greatly saves heat loss.
